68 year old man, with gradual darkening beneath the left eye over 1 year.
Has also noticed a small, round tumour located at edge of lower eyelid.

What diagnoses and treatments should be considered?

Spherical tumour, mm size, with telangiectases loc. to limbus of lower left eyelid.
Basal cell carcinoma must be suspected (telangiectases superficially on smooth skinned area).
- Irregularly pigmented, unsharply delimited macular lesion below left eye.
- Clinical lentigo with uneven pattern of pigmentation suspect for lentigo maligna melanoma.
- Biopsy confirms that this is a malignant melanoma in situ.
Treatment: Plastic surgery with skin excision of both forms of tumour.
