20-year old woman present with groups of sterile pustules occur in crops on one hand and both feet. The eruption is associated with thickened, scaly, red skin which easily develops painful cracks (fissures).
Which diagnosis and what treatment should be considered?

Palmoplantar pustulosis (PPP)
The condition varies in severity and may persist for many years. It is not known what causes exacerbations or remissions. It is however, much more common in smokers and among females. An association with thyroids dyscrin functions has been described. PPP is not infectious to other people.
Treatment
Treatment includes use of emoliens, topical high-potents steroids, coal tar, calcipotriol, acitretin, tetracycline, methotrexat or ciclosporin. The success of treatment is however difficult to predict. Stop of smoking is highly recommended.
